The ADHD Diagnosis Process in the UK Explained

Understanding the ADHD diagnosis process in the UK can help you know what to expect and make informed decisions about your care. This guide covers both the NHS pathway and the private route, with an honest comparison of the two.

Step 1: Recognising the symptoms

The first step is recognising that your difficulties may be explained by ADHD. Many adults reach this point after reading about ADHD, after a family member is diagnosed, or after years of struggling without explanation. Common triggers include a child being diagnosed with ADHD (prompting a parent to recognise the same traits in themselves), reading about ADHD on social media or in the press, or a mental health professional suggesting ADHD as a possible explanation.

Step 2: Speaking to your GP

Whether you pursue NHS or private assessment, speaking to your GP is a useful first step. Your GP can rule out other medical causes for your symptoms (such as thyroid problems, sleep disorders, or anaemia), be kept informed of your assessment outcome, and potentially refer you under the NHS Right to Choose if you wish to pursue this route.

You do not need your GP's permission to book a private ADHD assessment. However, keeping your GP informed is in your best interest for ongoing care.

Step 4: Completing pre-assessment questionnaires

Before your assessment, you will be asked to complete standardised rating scales. These typically include a self-report scale (such as the Adult ADHD Self-Report Scale or Conners Adult ADHD Rating Scale) and a collateral report completed by someone who knows you well. These questionnaires help your clinician understand the pattern and severity of your symptoms.

Step 5: The assessment itself

A comprehensive ADHD assessment includes a detailed clinical interview covering your current symptoms and their history, a review of your completed questionnaires, exploration of alternative or co-occurring conditions, and a discussion of the diagnostic criteria and how they apply to your presentation.

The assessment typically lasts 90-120 minutes and is conducted by a qualified clinician (psychiatrist, psychologist, or specialist nurse). ADHD must have been present since childhood for a diagnosis to be made, even if it was not recognised at the time.

Step 6: The diagnostic outcome

Following your assessment, you will receive a written report confirming or ruling out ADHD. If ADHD is confirmed, the report will specify the type (inattentive, hyperactive-impulsive, or combined), the severity, and the clinical reasoning. The report will also include recommendations for treatment and support.

Step 7: Beginning treatment

If ADHD is diagnosed, treatment options include medication, psychological support, and lifestyle adjustments. Your clinician will discuss the options with you and, if medication is recommended, will initiate a titration process to find the right medication and dose.

What makes a good ADHD assessment?

A comprehensive ADHD assessment should include a detailed clinical interview (not just questionnaires), consideration of alternative and co-occurring conditions, a written report with clear clinical reasoning, a letter for your GP, and follow-up support for treatment initiation. Assessments that rely solely on questionnaires without a clinical interview do not meet NICE guidelines and should be avoided.

NICE guidelines for ADHD assessment

NICE guideline NG87 (updated 2019) sets out the standards for ADHD assessment and management in the UK. Key requirements include a comprehensive clinical assessment by a specialist, consideration of co-occurring conditions, and a written report. All assessments at ADHD Assessment UK are conducted in accordance with NICE guideline NG87.
