What is included?
Your £39 monthly care fee includes:
- One routine private prescription approximately every 28 days for your agreed stable medication regimen.
- Up to 13 routine prescriptions during a 12-month period.
- Clinical review of your routine prescription requests.
- Monthly monitoring of medication effectiveness, adherence and potential adverse effects.
- Six-monthly review of your blood pressure, pulse and weight.
- One routine annual ADHD medication review when it becomes due while you remain enrolled.
- An annual treatment summary and medication letter for your GP.
- Reasonable communication with your GP about your ADHD treatment.
- Support with a Shared Care Agreement request.
- Routine medication-related questions through our secure patient system.
- Continuing specialist oversight if your GP does not accept shared care.
We aim to review complete routine prescription requests within three working days. Please submit requests at least seven days before your medication runs out.
What is not included?
The following are not included in the monthly fee:
- The cost of medication.
- Pharmacy dispensing, postage or delivery charges.
- Active titration or significant medication changes.
- Additional clinical appointments outside the included annual review.
- ECGs, blood tests, physical examinations or other investigations.
- Replacement prescriptions for medication that has been lost, stolen or used earlier than prescribed.
- Early prescriptions that are not clinically appropriate.
- Travel letters, occupational reports or other non-routine documents.
- Psychological therapy, counselling or ADHD coaching.
- Emergency or crisis care.
If your medication needs to be changed or re-titrated, we will explain any additional fee before arranging the appointment.
Who can join?
The plan is intended for adults who:
- —Are aged between 18 and 65.
- —Have completed ADHD medication titration with Avon Medical / ADHD Assessment UK.
- —Have reached a stable and clinically appropriate medication dose.
- —Have provided the required physical-health measurements.
- —Remain clinically suitable for private ADHD prescribing.
- —Have an NHS GP and consent to appropriate communication with their GP.
- —Live in an area where Avon Medical is authorised to provide the service.
- —Do not currently receive their ADHD prescriptions through an NHS Shared Care Agreement.
Patients diagnosed or treated by another provider must first complete Avon Medical's transfer-of-care assessment. Acceptance onto the plan is not automatic.
How does it work?
Complete titration
Your clinician will help you find a medication and dose that provides an appropriate balance between benefit and possible adverse effects.
Confirm eligibility
Once your medication is stable, your clinician will confirm whether the Continuing Care Plan is clinically appropriate.
Activate your plan
You will receive a secure link to read the patient agreement and activate the £39 monthly payment.
Complete your monitoring form
Before each prescription, you must provide accurate information about your medication, response, adverse effects and any relevant changes to your health.
Clinical review
A qualified prescriber reviews the information. A prescription is issued only when it remains clinically safe and appropriate.
Pharmacy payment
Your prescription is sent to the agreed pharmacy. The pharmacy will confirm the separate cost of your medication, dispensing and delivery before supplying it.
Important prescribing information
Payment for the Continuing Care Plan does not guarantee that a prescription will be issued.
Every prescription remains an individual clinical decision. Avon Medical may need further information, physical-health measurements or a clinical appointment before prescribing.
Prescribing may be paused when:
- —Required monitoring information has not been provided.
- —An annual medication review is overdue.
- —There has been a significant change in physical or mental health.
- —New medication interactions or contraindications are identified.
- —There are concerns about adverse effects, misuse, dependence or diversion.
- —Continuing treatment is no longer considered clinically appropriate.
Any decision to pause or discontinue prescribing will be explained, documented and managed in accordance with our clinical and patient-safety procedures.
What happens if my GP accepts shared care?
If your GP accepts a Shared Care Agreement, they will normally take over routine prescribing through the NHS. Your £39 monthly private-prescribing plan will then end.
You will still require ongoing specialist oversight and an annual ADHD medication review. Avon Medical's Shared Care Annual Review currently costs £149 when it becomes due.
Acceptance of shared care is entirely at the discretion of your GP practice and cannot be guaranteed.
Cancelling your plan
You can cancel the recurring payment before your next billing date through your patient account or by contacting enquiries@adhdassessmentuk.co.uk. There is no cancellation fee or minimum contract period.
Cancelling payment does not replace the need for a safe clinical handover. If you are taking prescribed ADHD medication, please contact us before cancelling so that we can discuss continuity of treatment and notify your GP where appropriate.
Your statutory consumer rights are unaffected.
